Every client that books an appointment must fill out these forms before being seen.

ALL PAYMENT INFORMATION MUST BE STORED ON FILE. DO NOT CHOOSE THE GUEST OPTION. IF CARD IS NOT STORED YOUR APPOINTMENT MAY BE CANCELLED. ONLY DEBIT/CREDIT CARDS ACCEPTED THROUGH SQUARE.
NO CALL NO SHOW OR SAME DAY CANCELLATIONS WILL BE CHARGED AT SCHEDULED TIME OF APPOINTMENT.
FACESNMORE WILL ALLOW EACH CLIENT TO RESCHEDULE TWICE. ANY CANCELLATIONS SCHEDULED AFTER THE LAST CANCELLED APPOINTMENT WILL BE CANCELLED BY OUR OFFICE.
All fees are included in salon prices. Cancellations must be given a 48 hour notice. If no notice is provided your payment will be charged in full. If you are more than 10 minutes late for your appointments you may be turned away and asked to reschedule for the next available appointment.
